Who qualifies for Home visits?

Our home visit program is designed for patients who cannot easily travel to a medical office because of their health, mobility, or functional status.

You may qualify if you experience

Difficulty walking or transferring
Frequent falls or high fall risk
Shortness of breath with minimal activity
Cognitive impairment such as dementia
Advanced chronic illnesses (heart failure, COPD, cancer, kidney disease, stroke, etc.)
Dependence on others for daily activities
Significant fatigue, weakness, or frailty
Heavy caregiver burden or lack of transportation

If traveling to a clinic has become stressful, exhausting, or unsafe, home visits may be the right level of care.

What is Complex Care Medicine (Palliative Medicine) ?

Specialized medical care for people living with serious illness who are not ready for Hospice. Focus on relief of symptoms, stress, and quality of life. Provided at home by a nurse practitioner

Did You Know?

A landmark 2010 NEJM study (Temel et al.) in patients with cancer showed that those receiving early Complex Care Medicine (Palliative Medicine) had better quality of life, fewer depressive symptoms, less aggressive end-of-life care — and lived almost 3 months longer than those who received standard oncology care alone.

Research in advanced dementia shows that patients who receive Complex Care Medicine (Palliative Medicine) have better comfort, fewer unnecessary hospitalizations or aggressive interventions, and less stress for families and caregivers. (Eisenmann et al., 2020; Volicer et al., 2015)

ADAPT Trial of 306 adults with COPD, heart failure, or interstitial lung disease at high risk of hospitalization or death found clinically meaningful improvements in quality of life at 6 months for those who receive Complex Care Medicine (Palliative Medicine) compared with usual care
